• Propranolol is a non-selective β adrenergic blocker without intrinsic sympathomimetic activity.
• It is a competitive antagonist at β adrenergic receptors.
• It blocks ß1 and ß2 receptors but has weak activity on ß3 subtype.
• It is also an inverse agonist. It reduces resting heart rate as well.
• Propranolol is a white crystalline powder, almost odourless and bitter in taste.



Pharmacological Actions of Propranolol (Prototype of β-Blockers):
(i) Propranolol decreases heart rate, force of contraction and cardiac output (c.o.).
(ii) It has direct depressant effect on heart due to membrane stabilizing action at higher doses.
(iii) In angina patients, it improves oxygen supply / demand status.
(iv) It decreases renin release from kidney ( β1 mediated) causing a fall in BP.
(v) On prolonged administration of propranolol BP gradually falls in hypertensive subjects but not in normotensive.
(vi) It can cross the blood brain barrier. It produces minute behavioural changes, forgetfulness, increased dreaming and nightmares with long-term use of relatively high doses. It suppresses anxiety in short term stressful situations.
(vii) Propranolol is a potent local anaesthetic, but is not clinically used for this purpose because of its irritant property.
(viii) Prolonged propranolol therapy may reduce carbohydrate tolerance by decreasing insulin release.
(ix) Plasma triglyceride level and LDL/HDL ratio is increased during propranolol therapy.
(x) Instillation of propranolol reduces secretion of aqueous humor, intraocular tension is lowered.
Pharmacokinetics: Propranolol is well absorbed after oral administration but has low bioavailability due to first pass metabolism in liver. It is lipophilic and penetrates into brain easily.

• Propranolol is indicated for hypertension; angina pectoris; cardiac arrhythmias; myocardial infarction; CHF; glaucoma.
• It is a Class II antiarrhythmic drug.
• It is used as prophylaxis of migraine, thyrotoxicosis, anxiety.
• Propranolol is contraindicated in bronchial asthma, COPD, bradycardia, hypotension, Prinzmetal angina.
• Propranolol brand names: Inderal, InnoPran XL.
